Joint pain, arthritis, muscle stiffness, neck pain, shoulder pain, back pain, knee pain, and sciatica can make even simple daily activities challenging. Whether your discomfort developed after an injury or gradually over time, understanding the underlying cause is the first step toward lasting relief.
In Traditional Chinese Medicine (TCM), many painful musculoskeletal conditions are categorized as Bi Syndrome, also known as Painful Obstruction Syndrome. Rather than focusing only on where you hurt, Chinese Medicine seeks to understand why the pain developed and how to restore balance throughout the body.
At MJG Acupuncture & Massage, Marc provides personalized acupuncture treatments for patients throughout Flatiron, Union Square, Gramercy, Chelsea, Manhattan, Prospect Heights, Crown Heights, Brooklyn, and the greater New York City area. Every treatment plan is individualized because no two people experience pain in exactly the same way.
What Is Bi Syndrome?
In Traditional Chinese Medicine, Bi Syndrome describes pain that develops when the normal flow of Qi (energy) and Blood becomes obstructed. This obstruction may contribute to pain, stiffness, numbness, heaviness, swelling, or limited movement.
Traditional Chinese Medicine recognizes that external factors such as wind, cold, dampness, and heat may influence how pain develops and presents. While these concepts differ from Western medicine, they help guide an individualized diagnosis and treatment plan.
Many people diagnosed with arthritis, tendonitis, chronic muscle tension, frozen shoulder, sciatica, repetitive strain injuries, or sports injuries may also fit one or more Bi Syndrome patterns from a Traditional Chinese Medicine perspective.
Common Symptoms of Bi Syndrome
Symptoms vary from person to person but commonly include:
Joint pain
Arthritis discomfort
Muscle stiffness
Neck pain
Shoulder pain
Knee pain
Hip pain
Low back pain
Sciatica
Numbness or tingling
Heaviness in the limbs
Swelling
Pain that moves from one joint to another
Pain that worsens during cold or damp weather
Limited range of motion
Understanding your unique symptom pattern allows your acupuncture treatment to be customized specifically for your needs.
The Four Main Types of Bi Syndrome
Wind Bi
Wind Bi often presents as pain that moves from one area of the body to another. One day the shoulder may hurt, while the next day the discomfort shifts into the elbow or wrist.
Cold Bi
Cold Bi typically causes intense, fixed pain that improves with warmth and becomes worse during cold weather or exposure to air conditioning.
Damp Bi
Damp Bi is often associated with heaviness, swelling, stiffness, and aching joints. Many patients notice their symptoms become worse during rainy or humid weather.
Heat Bi
Heat Bi may present with inflamed, swollen, warm joints accompanied by redness or burning sensations.
Many patients experience a combination of these patterns, making a comprehensive evaluation especially important.
Questions Your Acupuncturist
One of the strengths of Traditional Chinese Medicine is that it looks beyond the symptom itself. During your initial consultation, I ask questions designed to understand your complete health picture.
Where is your pain located?
The location of your discomfort helps identify the muscles, joints, connective tissues, and Traditional Chinese Medicine meridians involved.
How would you describe your pain?
Is it:
Sharp?
Dull?
Aching?
Burning?
Constant?
Intermittent?
Fixed?
Moving?
Each quality provides valuable information that helps guide treatment.
Does weather affect your symptoms?
Many patients notice increased discomfort during:
Cold weather
Rain
Humidity
Sudden weather changes
These observations often help identify the underlying Bi Syndrome pattern.
What makes your pain better?
Does it improve with:
Heat
Gentle movement
Stretching
Rest
Massage
Acupuncture
Understanding what relieves your symptoms helps personalize your treatment.
What makes your pain worse?
Examples include:
Sitting too long
Standing
Exercise
Stress
Cold temperatures
Repetitive movements
These details help identify contributing factors.
Do you experience other symptoms?
Pain rarely exists in isolation. Additional symptoms may include:
Fatigue
Poor sleep
Digestive concerns
Stress
Anxiety
Numbness
Tingling
Swelling
Traditional Chinese Medicine evaluates the entire person rather than focusing solely on one painful area.
How is your overall health?
We'll also discuss:
Sleep
Nutrition
Exercise
Stress levels
Previous injuries
Medical history
Lifestyle habits
All of these factors help create the most appropriate treatment plan for you.

Question to determine what kind of Pain Syndrome in Chinese Medicine
Understanding the type of bi-syndrome (pain syndrome) you may have is essential to acupuncture treatment. As an acupuncturist, Marc Gian will diagnose your condition to devise the most effective treatment strategy. To this end, he asks a series of questions to determine the type of bi-syndrome affecting you. These questions will provide a comprehensive view of your health and symptoms, contributing to a highly personalized holistic treatment.
Essential questions Marc will ask to decipher your type of bi-syndrome (pain syndrome):
What specific areas of your body are affected by Pain? The location of the Pain can help identify the meridians or energy channels involved, which can help pinpoint the acupuncture points and massage techniques to be used during treatment.
Can you describe the nature of your Pain?
Is the Pain dull, sharp, constant, intermittent, moving, or fixed in one spot? The quality of Pain can offer insights into the type of bi-syndrome, such as whether it's associated more with wind (moving pain) or dampness (heavy, fixed pain).
Do weather changes affect your Pain?
In bi-syndrome, the Pain often varies with weather conditions. For instance, Pain that worsens with cold weather might indicate a cold bi-syndrome, while damp weather may be associated with a damp bi-syndrome.
Do you have any other symptoms associated with the Pain?
Symptoms like numbness, tingling, heaviness, or swelling can provide further clues to the nature of the bi-syndrome.
What makes your Pain better or worse?
Does rest alleviate the Pain? Does it get worse with activity? Is it better with heat or cold? Understanding what exacerbates or eases your Pain helps create a tailored treatment approach.
Do you have any accompanying symptoms, such as fatigue, stress, or poor digestion? These could be signs of an underlying imbalance in the body that is contributing to your bi-syndrome.
What is your overall energy level?
In TCM, fatigue or low energy can signal a deficiency in Qi or blood, possibly related to your pain symptoms.
What is your lifestyle like? What are your diet, exercise, and sleep habits? These factors can affect the balance of Qi and blood in your body, influencing your bi-syndrome symptoms.
Remember, these questions are meant to help us better understand your individual health condition and the specific bi-syndrome affecting you. By answering these questions honestly and thoroughly, you help us create a customized treatment plan for your pain management. Your journey to health and wellness is a partnership, and we value your active participation in this process.
Traditional Chinese Medicine Herbal Formulas and Essential Oils
Various Herbal Formulas and Essential Oils and Chinese Medicine Topicals can be used to address specific pain symptoms. One of the most effective aromatherapeutic blends is from Merdian Biologix, a brand of essential oils called Quiet Channel. Quiet Channel Essential Oils Blend can be massages in the muscles and applied on acupuncture points! It consists of a balanced blend of essential oils to alleviate pain due to cold and worse in cold, damp weather. A fundamental oil in this blend is Lemongrass. Lemongrass is a warming and moving oil known as the "tendino-muscular oil" and will warm the area to alleviate pain.
